How Do Eye Drops Control Glaucoma?
The main goal of glaucoma treatment is to lower intraocular pressure (IOP) to preserve the optic nerve. And that is exactly what hypotensive eye drops do. They generally work through two main mechanisms:
- Decreasing fluid production: The eye constantly produces aqueous humor. Some eye drops "slow down" this production, lowering internal pressure.
- Facilitating drainage: Other eye drops act on the trabecular meshwork or alternative pathways, making it easier for the accumulated fluid to drain out of the eye.
There are several classes of medications, such as Prostaglandin Analogs, Beta-blockers, Alpha-adrenergic Agonists, and Carbonic Anhydrase Inhibitors. The ophthalmologist selects the most appropriate one (or a combination) by evaluating not only the eye pressure but also the patient's systemic history (such as asthma or heart problems, which may contraindicate certain eye drops).
Side Effects and Low Adherence
One of the biggest challenges in controlling the disease is not the efficacy of the medication, but rather patient adherence. As we know from reading about glaucoma symptoms, the disease is silent at first. The patient does not feel pain or visual worsening and, therefore, often forgets or is discouraged from using the chronic medication daily, which causes dangerous fluctuations in pressure.
